Harvard Street Neighborhood Health Center is Hiring a Nurse Practitioner Near Dorchester, MA

PATIENT CENTERED MISSION: We at Harvard Street Neighborhood Health Center strive to put our patients at the center of our practice. Our patient care team is made of both clinical and non-clinical members and unites departments and services across the clinic to ensure that safe, effective, and efficient whole-person care is provided that is respectful and responsive to individual patient preferences, needs, and values.

SUMMARY:

Diagnoses and treats variety of diseases and injuries in general practice by performing the following duties under Physician Supervision

DUTIES AND RESPONSIBILITIES:

  • Evaluates and treats patients (including walk-ins)
  • Provides preventive health care advice and case management, assesses psychological impact of illness and treats or appropriately refers patients, for evaluation and treatment.
  • Orders or execute various tests, analyses diagnostic images to provide information on patient’s condition.
  • Administrates and prescribes treatments and drugs.
  • Inoculates and vaccinates patients to immunize them from communicable disease.
  • Elicits and record information about patient’s medical history.
  • Participates in the Medical Student and/or other clinical discipline training activities at the Center.
  • Participates in the formation and evaluation of applicable protocols.
  • Maintains continuity of care when Health Center patients utilize other health care facilities, i.e. referrals, emergency room services, outpatient departments, specialty clinics, and inpatient services.
  • Refers patients for specialty services, as appropriate, provides documentation according to Health Center procedures and follows up on results.
  • Participates in staff meetings, including quality assurance.
  • Participates in 24-hour on-call medical coverage, including at a minimum, two consecutive nights per week and one weekend every four weeks.
  • Participates in on-site and off-site outreach and continuing education programs and ensures training to maintain license and to meet the needs of the population.

Minimum Entrance Qualifications:

Ø FNP Degree

Ø Must have current DEA and MA licenses.

Ø Prior experience in ambulatory setting preferred.

Ø Experience in diverse culture preferred.

Ø Bilingual in Haitian creole and/or Spanish a plus

WORKING CONDITIONS:

The FNP works in a Boston’s inner city community health center in. Occasional Saturday and/or weeknight coverage is an essential function of this position. On-call telephone coverage will be required, frequency depends upon specialty Long hours are sometimes required.

PHYSICAL REQUIREMENTS:

Physical demands requiring lifting of small pieces of equipment, boxes, and/or assisting with patient lifting, not to exceed 50 pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with or without assistive audio devices.
